Specialty Areas and Chief Mentors
The School of Optometry at the University of California, Berkeley offers Residencies in the following areas:
Application
To apply, applicants must register on-line with the Optometric Residency Matching Service, Inc. (ORMS) and follow its
application guidelines. The deadline for applying to ORMS will be listed on their website: www.orms.org.
Register on-line with the Graduate Division at U.C. Berkeley by completing the application at:
sis.berkeley.edu/grdappl/grd_login_menu/
Click
on "Create an Online Application Account." For Step 2, choose "Fall 2008." You only need to fill
out Form A (pages A1, A2, and A4 only). On page A2, pick "Optometry Cert (Cert-Opt)" as your proposed
department and degree. On page A4, include both your undergraduate and optometry school information. Do not fill out
Form B, C, D, E, F, G, GW, or H. The $60 application fee has been waived (you do not need to complete a waiver) and
the GRE is not required. This website may not be active until the fall.

Supporting Documents
The deadline for submission of supporting documents is January 23.

A copy of your Curriculum Vitae and and a 1-page letter of intent

Three letters of recommendation

Results of NBEO Part I and II and TMOD. Students may submit their printouts of part I for the initial application, and forward part II scores when they are received.

Official transcripts of all college-level course work, including undergraduate, graduate, and optometry
school.
Applicants must satisfy all requirements to allow for California licensure. This includes successful completion
of NBEO Part III and passing the California Law Exam administered by the NBEO in April. Candidates must register
to take the California Law Exam with the NBEO. Their deadline for registration is in February. Details about
California licensure, and a copy of the California law can be found at:
www.optometry.ca.gov.
Applicants must have graduated from an Accreditation Council on Optometric Education (ACOE) accredited school
or college of optometry, and have earned an OD degree by the time of matriculation.
It is recommended that applicants have achieved at least a 3.0 (on a 4.0 scale) cumulative GPA in the optometry
professional curriculum.
All applicants will be evaluated without regard to gender, race, color, religion, age, marital status, sexual
orientation, or national origin. The Optometric Residency Committee, chaired by the Dean, will follow the
procedures of the University of California as an equal opportunity employer.
There are a limited number of positions and only the most qualified candidates will be admitted.
On-campus applicants are welcome to visit the campus and clinics on an informal basis. Candidates with completed applications
may be invited by the Residency Admissions Committee to the campus for a formal interview.
Candidates are strongly encouraged to submit their application materials early to facilitate the scheduling of
invited interviews.
Requirements for Awarding of Certificate
Four case presentations, one formal paper, patient logs, completion of binder, required evaluations, and favorable recommendation from the Chief Mentors.
VA Applicants
VA applicants must apply on-line through the Optometric Residency Matching Service, Inc. (ORMS) and follow its
application guidelines. Please check the ORMS website to confirm their deadline which is usually Feb. 1:

Applying to More than One Program:
Four VA programs are affiliated with UC Berkeley School of Optometry.
If you are interested in applying
to one of these programs as well as to an on-campus program, you will need to send your application materials both
to Tamara Baltar at UC Berkeley as well as to the Program
Coordinator(s) of the VA(s) in which you are interested. If you are applying to an on-campus program and a VA
program, you only need to fill out the on-line grad division application once. If you are applying only to a VA
program, and not to an on-campus program, you do not need to fill out the on-line grad division application.
Please contact the VA Program Coordinator directly to get more information about their programs and application
materials.
Candidates applying to affiliated VA programs must be US citizens at the time of the ORMS match, and if male, must have registered for Selective Service as required by the federal government.
Final candidates will be selected by the mentors of each specialty area in conjunction with the Residency
Supervisor, the Residency Director, and the Optometric Residency Admissions Committee.
The Graduate Division UC Berkeley has final approval of all applicants recommended for admission.
