Application Form

The NEI Summer Research Program Application Form is available in three formats for downloading. Select the preferred format, complete the form, and mail it as an attachment to Dr. Wildoset: wildsoet@berkeley.edu.

Please note that you should include your resume (200 words or less) within the resume field on the form (i.e., do not mail a separate resume).

Microsoft Word

• Download the MS Word form, fill in the boxes (Text Form Fields), save in the Word (.doc) format with your last name included in the file name (e.g., Smith_nei_applic.doc), and email it as an attachment to Dr. Wildsoet.

RTF (Rich Text Format)

• Download the RTF form, fill in the boxes (Text Form Fields), save in the RTF (.rtf) format with your last name included in the file name (e.g., Smith_nei_applic.rtf), and email it as an attachment to Dr. Wildsoet.

PDF

• If you have Adobe Acrobat, you can enter your responses in the PDF form and save in the PDF (.pdf) format with your last name included in the file name (e.g., Smith_nei_applic.pdf), and then email it as an attachment to Dr. Wildsoet.

• Note: Although the PDF form can be displayed in Adobe Reader and data can be entered for printing, you cannot save the form in Reader (only in Acrobat 6.0 or later, or an equivalent PDF editing application).
